Office 365, Canvas, Library, Myhealth, Password Reset and Self-Service

This form is for students who are new applicants or registered for classes in an upcoming/current term at EVC or SJCC. You may request support for accessing or using Office 365, Canvas, Library, MyHealth, Password Reset and Self-Service.  For an Office 365 account password reset, you have the option to reset or change your password at https://sso.sjeccd.edu. Due to security measures associated with servicing your request, we kindly ask that you provide the following additional information:

Full name, Student ID and Username, BirthDate (Month and Day), Personal email, Zip code, Old and new phone number (if Applicable), What type of help do you need?
