The R-40 form is used for high school students to enroll in college-level courses at EVC and SJCC through their respective Concurrent Enrollment or Dual Enrollment programs.

For SJCC: the R-40 form can be found here.

For EVC, to find the R-40 form:

  1. Click on this link

  2. Select Special Admit or CCAP Form

  3. Choose the appropriate form based on your student status (High School, Middle school, GED students).
